VERIFICATION POLICY

Fostering Success Act, Inc. (“FSA”) is a nonprofit organization that facilitates and may fund the provision of services to eligible youth and young adults pursuant to the provisions of OCGA § 48-7-29.24, the Fostering Success Act.  By submitting a request to FSA, you agree that we may use the information you have provided to enable us to determine your initial and ongoing eligibility for services in accordance with the law.

Under the Fostering Success Act law, FSA is required to take steps to ensure that each applicant is eligible to receive services authorized by the law.  Those requirements vary, but they may include you to demonstrate that you are (1) A current or former foster youth who is or was in the custody of the Georgia Division of Family and Children Services (DFCS) within the applicable age ranges, or a “justice-involved youth” who is currently or was previously committed to the Georgia Department of Juvenile Justice (DJJ) with placement in a community-based or nonsecure setting; (2) that you are enrolled in an approved educational or vocational program; and (3) that you are receiving services through an approved provider.  You may view the eligibility requirements here https://fosteringsuccessact.org/law

To accomplish our verification obligations, we may need to speak with officials from Georgia’s foster care or juvenile justice agencies such as Georgia DFCS or DJJ; with foster care or group home providers; with foster organizations, with foster or adoptive parents; and/or with your educational, vocational, and community service providers.

By submitting this form and applying for services, I hereby agree that FSA may use any and all information I provide as well as information about me that they may gain from other sources to verify my eligibility for the FSA services I am requesting as well as other FSA-related services for which I may be eligible.  I hereby authorize FSA to communicate on my behalf with, provide information to, and receive information from any and all agencies and providers as may be necessary to verify my eligibility, including but not limited to the Georgia Division of Family and Children Services (DFCS) and the Georgia Department of Juvenile Justice (DJJ), and any and all agencies that contract with DFCS and DJJ.   

I have read and agree to the verification policies and by typing my name on the previous page, I give my consent and assent to the release and exchange of my records and information by and between FSA, DFCS, and DJJ for such purposes. This authorization to release and exchange information about me shall remain in force until I have revoked same in writing.  

Georgia tax filers can apply to receive 100 percent tax credit for donations made to qualified foster support organizations.
